Dr. MJ Gonstead is the only provider in our office in network with Blue Cross Blue Shield. Many Blue Cross plans have out of network benefits, but out of network chiropractic services apply to your out of network deductible. If you have questions about your deductible or benefit details, you can find that information by calling your plan using the phone number on your insurance card. If you are unsure about the benefits after calling Anthem, feel free to ask our office staff. They can help you determine your benefits and which route is best for you.
Anthem Blue Cross does not require any additional paperwork beyond our standard new patient paperwork.
